Rehabilitation and Wellness
Physical Therapy
Our personalized physical therapy supports residents to gain or regain maximum mobility. Physical therapists use their expertise to work with residents either towards recovery or preventatively to help decrease the risk of an injury occurring or recurring. At Summit Rehabilitation and Care Community, our rehabilitation therapy is recovery-focused to create a smooth transition home.
Occupational Therapy
Summit Rehabilitation and Care Community offers occupational therapy to support residents with daily activities to help them maintain their independence. Therapists work with residents in their environment, making adjustments as needed, thereby ensuring personal goals are achieved.
Speech and Language Therapy
For those who have suffered a stroke, brain injury, or another health issue that has affected their communication ability and/or swallow functionality, Summit Rehabilitation and Care Community offers the services of a Speech-Language Pathologist (SLP). SLPs assess communication and swallow difficulties and provide skilled interventions that help improve cognition, speech production, fluency, language, voice, resonance, hearing, and all aspects of swallowing.
Respiratory Therapy
Respiratory Therapy specializes in providing healthcare for your lungs and can benefit those with asthma, pneumonia, emphysema, COPD, lung disease, lung training, and other related medical conditions.
Skilled Nursing Care and Chronic Disease Management
Our nursing services at Summit Rehabilitation and Care Community work in conjunction with the medical needs of the individuals we care for. We formulate a unique plan of care catered to the specific needs of every individual that stays with us.

Summit Rehabilitation and Care Community specializes in support for individuals living with Alzheimer’s and other forms of dementia through our specialized programming.
Our individualized person-centered Dementia Care services are designed using each resident’s life history as a foundation to provide therapeutic programming throughout the day. These activities may include:
- Mental Workout – Mental exercises involving discussions, brain-stimulating games, and reminiscing.
- Physical Activity – A variety of exercise opportunities, including group exercises, walking programs, and dancing.
- Creative Expressive – Personalized activities to engage the senses and express thoughts, opinions, emotions, and beliefs.
- Life Skills – Individual or group activities that provide a sense of purpose, belonging, connection, and meaning.
Respite Care
Summit Rehabilitation and Care Community offers respite care for recovery and caregiver support. During their stay with us, individuals receive care and support in a safe, comfortable environment that promotes well-being. Our respite care services provide caregivers with peace of mind knowing their loved one is being taken care of and receiving compassionate and individualized care.

Hospice and Comfort Care
Our End of Life programming and comfort care services, including hospice and palliative care, focus on symptom management, pain relief, and end of life wishes. Team members at Summit Rehabilitation and Care Community customize care plans for each individual and do everything they can to ensure residents live comfortably during their last chapter of life.
